Sometimes, even if a person lives a healthy lifestyle, abstains from alcohol or drinks it modestly, they can experience a condition that mirrors gout, but is in actuality, a different arthritic condition altogether known as pseudogout. To help you understand the differences between these two, the following is a breakdown of the causes, symptoms and treatments for each condition.

Gout
Gout is a form of arthritis that occurs suddenly, usually affecting the big toe, and causes severe painful attacks, tenderness and redness in the affected joint. An attack can last days or weeks. Gout is caused by a build up of uric acid which turns into uric acid crystals that are deposited into the joints causing them to become inflamed. The first attack of gout often occurs late at night.

A build up of uric acid that develops into gout can result from:
?High alcohol consumption
?Eating excessive food rich in purines such as organ meats
?Crash diets
?Joint injury
?Surgery
?Chemotherapy

The symptoms of gout include:
?Sudden intense joint pain

Pseudogout
Pseudogout is a form of arthritis that occurs when calcium pyrophosphate crystals accumulate in joints. Although it is sometimes referred to as calcium pyrophosphate dihydrate deposition disease (CPDD), the disease is called pseudogout due to the fact that its symptoms and crystal build up are similar to gout.

Like gout, pseudogout causes a sudden extreme attack of pain and swelling to specific joints, which can last for days or weeks. However, unlike gout that usually attacks the big toe first, pseudogout is usually present in the knees, but can also affect the shoulder, elbows, wrists, hands, or ankles. Furthermore, pseudogout can become a chronic arthritic condition that feels more like osteoarthritis or rheumatoid arthritis.

It is clear that the development of calcium pyrophosphate crystals is what causes Pseudogout. Eventually, crystals can be deposited into the cartilage of a joint and then begin to damage it, a condition known as chondrocalcinosis. Although medical researchers are not sure why the calcium pyrophosphate crystals occur, it is suspected that the condition may be hereditary or related to:
?Hypothryroidism (underactive thyroid)
?Hemochromatosis (too much iron storage)
?Overactive parathyroid gland
?Low levels of magnesium in the blood
?Hypercalcemia (too much calcium in the blood)
?Joint injury
?Surgery (either related to a joint or not)

Unlike gout, pseudogout is not necessarily gender specific or diet related, and usually affects 3% of people in their sixties and approximately 50% of those in their nineties.

The symptoms of pseudogout include:
?Swelling of the affected joint
?Severe pain
?Warmth
?Redness

Treatment for pseudogout includes:
?NSAIDs
?Cortisone injections
?Ice therapy, mild exercise and rest
?Surgery ? only occurs if severe damage has been done to the joints.

Gout develops when the level of uric acid in the blood is too high. In the past it was known as the patrician's malady because it was considered a diseased of over indulgence amongst the wealthy. High purine diet can affect gout and red meat is high in purine. The rich ate red meat while the poor could only afford grain.

Gout is predominantly a male disease up to the age of 50. It starts around 30 and the peak is between 40 and 50 years. It is rare in pre-menopausal women. Hippocrates wrote about gout as a disease of males and post-menopausal women and urate crystals were found in the big toe of an Egyptian mummy.

Gout can be classified as primary and secondary. In primary gout the body either produces too much uric acid or it cannot excrete it effectively to maintain a normal blood level. These patients benefit significantly from dietary changes. Secondary gout develops as a result of other factors. Some medical conditions can raise uric acid level and trigger gout. These include renal disease and myeloproliferative disorders. In leukaemia, the imbalance is due to excessive production of uric acid, and in renal disease the excretion of uric acid is impeded. Drugs can affect blood uric acid levels. These include diuretics especially thiazide diuretics, aspirin and cytotoxic drugs. Gout is also linked to obesity and alcohol.

Acute gout usually develops during the night. 70-90% of acute gout develops in the big toe and typically affects the first metatarso-phalangeal joint. This is called podagra. Within hours the pain becomes severe and throbbing. The big toe swells up and can feel hot. It is extremely painful to the touch and even the touch of the bed sheath can induce excruciating pain. The acute episode resolves on its own but can take over a week. Medication can shorten this time.

Since the introduction in 1951 of probenacid and allopurinol in 1963, chronic gout is rare apart from non-compliant sufferers. Before these drugs were introduced chronic gout caused severe distress amongst sufferers. Acute gout was frequent and severe. In addition urate crystals separated out into other joints, kidneys and other organs leading to polyarthritis, kidney stones, heart disease and tophi.

Diet and weight loss has an important role in gout management. A low intake of purine can reduce blood urate level by up to 20%. Foods high in purine are meat especially liver and other organ meat, sausages and other processed meat, seafood, milk, eggs and certain beans. Alcohol increases the possibility of gout. Beer is the worse culprit. It contains guanosine. This chemical is converted by the gut flora into uric acid.

Today, with screening and tests done for insurance purpose, we are seeing a group of individuals with high uric acid without any signs of gout. These people need to be monitored. Once all potential factors that can influence gout are eliminated or reduced and diet and weight loss in place, if the uric acid level remains high, then maintenance therapy in the form of allopurinol is recommended to keep the blood uric acid level within the normal range. This stops urate crystals separating out into important parts of the body.

Gout can be very painful so it's no wonder that anyone suffering from it would seek an immediate gout remedy. Here are some main stream medical as well as natural remedies.

Side Effects

Drugs do have their side effects. Allopurinol, for example, can cause skin rashes, blood vessel inflammation or liver toxicity. It can also weaken the kidneys. Colchicine is also an effective drug, but on its bad side, it can cause nausea, weakness, vomiting, hair loss, numbness and abdominal pain.

Corticosteroids, while providing immediate relief through an injection, can also cause similar side effects and are therefore not recommended for prolonged use. Corticosteroids can also cause joint and cartilage weakness.

Home Gout Remedies

Some gout patients find that the symptoms of gout may be reduced by performing simple tasks at home or in the office. These include elevating the affected joint, preventing blood from rushing into the area and cause further swelling. Resting the affected joint or immobilizing it can also bring about a certain level of relief.

Another remedy that has proven successful in alleviating pain is through physical and occupational therapy. Most gout patients suffer needlessly from pain because of improper use of the joints. Both therapies can train their muscles on the proper way of walking and standing. Massages can also help promote relaxation and a feeling of well being.

Relaxation techniques have also shown much promise in reducing pain and helping the muscles relax. Most gout patients also report less incidents of swelling pain after they reduced emotional and physical stress.

Contrary to some, hot compresses do not aid in reducing the symptoms of gout; it can produce temporary relief, but heat increases blood circulation in the affected area and can irritate it. Applying ice packs may also cause more uric acid crystals to form.

There are devices that may be used to make day-to-day tasks easier and much more comfortable: canes, carts, grab bars and such may be utilized to aid in the performance of duties and to conserve precious energy. Knowing that things can be handled can also do a lot for one's peace of mind.

Another remedy to avoid further problems with gout is a lifestyle change. This is more permanent and while there are no guarantees, it can produce some very significant improvements in a patient's condition. By losing weight and having a proper diet, gout patients are able to control the occurrence of gout by simply avoiding foods that promote it, including animal protein sources such as red meats, meat organs, seafoods, shellfish and alcoholic drinks.

The only drawback to avoiding these foods is that most patients will miss out on the essential vitamins and minerals that they contain. To correct this, alternative sources of protein may be used such as tofu and low-fat dairy products. Dietary supplements may also be required not just to maintain the recommended levels of both vitamins and minerals in the body but also to promote the regulation of uric acid levels.

To promote the proper excretion of uric acid in the blood, gout patients are advised to increase their daily fluid intake or maintain it at 8 to 9 glasses. Water helps produce urine, which flushes out toxins and keep uric acid levels to a manageable level.

Alternative Remedies

Gouty Arthritis - Do You Have It?

The joints in your big toe have been hurting really bad, and you?ve noticed that it has turned warm, red, and seems to be swelling. For a while, you ignore the pain, and thankfully it disappears for a while, only to come back at odd times with a vengeance. If you?re no longer too young, but just young at heart, then chances are, you may want to go see your doctor and ask if you might have symptoms of gouty arthritis.

This kind of arthritis is just one of the more than 100 types of diseases in this category. However, gouty arthritis is one of the most painful among these and is caused by needle-like crystals of uric acid that form in your joints (usually, in this disease, the big toe, although other joints can be affected as well, like the heel, elbow, knees, wrists, fingers, etc.). So, if you can imagine those ?needle-like crystals? pricking your joints, then you can understand why there is so much pain involved in this disease.

Gouty arthritis has four stages, namely:

1.Asymptomatic ? At this stage, the person may have tested to have elevated levels of uric acid, probably in a general medical screening, but is not experiencing any pain in any joints. The doctor usually will not prescribe any treatment at this stage, but may suggest some diet changes to prevent the condition from worsening.

2.Acute Gout ? At this stage, the person begins to feel pain, and swelling and redness of the joints, leading to what is called as a ?gouty attack?. These attacks can happen intermittently. It is a good idea to visit your doctor once you feel any acute pain in the joint area to prevent the disease from progressing.

3.Interval ? This is the ?interval? between gouty attacks, when a person is not experiencing any pain at the moment. Many people mistakenly think that the worst is over when they are in an interval stage and refuse to visit the doctor. What they don?t know is that this is exactly what it is named, an interval, and there may be an attack waiting just around the corner.

4.Chronic Gout ? When there is no or very short interval between attacks, and there is already permanent damage to the joints. Constant pain medication is needed for this type of gout and it is essential that a physician oversee this disease at this stage. If proper treatment is adhered to in the Acute Gout stage, then one may never progress to this stage.

How is Gouty Arthritis diagnosed?

If you think you are already are experiencing the attacks, when you visit the doctor, then chances are, he may ask for any one or all of the following tests:

1.Synovial Fluid Analysis ? Synovial (joint) fluid is extracted from your joints through a sterilized needle inserted in the space. The fluid has a straw-like color and is then analyzed in a lab. Normally, joint fluids that look cloudy or is thick may be abnormal.

2.Uric Acid Test ? This is done through a simple blood test taken from a vein or capillary. The blood is then tested for levels of uric acid. If it is high then it will confirm gouty arthritis

3.Joint X-ray ? An x-ray of the joint may also be needed to properly see how the arthritis has progressed in the area.

How is Gouty Arthritis treated?

Treatment of gouty arthritis is usually geared toward immediate relief and stopping the pain and inflammation that come with the attacks. Medication is also given to prevent future attacks.

One of the drugs prescribed to reduce the pain, inflammation and swelling is Colchicine and often makes the pain settle in 12 to 48 hours. This medication decreases the inflammation and therefore the pain, but it does not alter the uric acid levels in the blood, although daily use of it helps to prevent subsequent attacks.

Over-the-counter pain relievers can also be effective if it is taken at the onset of the pain.

At times, a diet low in purines is given to lessen the uric acid levels. Some foods that may have to be abstained from are beef, pork, poultry, fish, shellfish, gravy, soda pop, beer, wine, etc.

Gout is an arthritic condition that occurs primarily on the feet, although it certainly can manifest itself in other parts of the body. It affects about 3% of the population and is predominantly a male disorder with only 5% of the cases affecting females, generally post menopause.

Gout is associated with hyperuricemia, which is an increased amount of uric acid crystals circulating through the blood, however, not all people with hyperuricemia experience gout attacks.

Unlike osteoarthritis or the "wear and tear" arthritis we all suffer, the symptoms of gout are different in that it is manifested by an acute onset of severe pain, redness and swelling, many times for no apparent reason. Many patients will complain of a sudden onset of pain occurring early in the morning while asleep. The theory behind this is that when sleeping, the feet are elevated and there is a slight diminishment of blood to the feet which in turn decreases the temperature of the feet just enough for the uric acid to crystallize in the joint of the foot and create pain.

Other factors that may precipitate a gout attack include local trauma, including foot surgery, with an acute medical illness, or following a high purine (protein) diet or alcohol binge.

Gout attacks most commonly occur in the foot, primarily in the big toe joint. The next most common locations are the middle of the foot followed by the ankle joint. They can also occur at the attachment of tendons such as the Achilles tendon into the heel. It is possible to have gout attacks in more than one area at the same time.

Diagnosis is generally on clinical appearance and the presenting history because it is not uncommon for a patient's uric acid level to be normal during an attack. Colchicine is the drug of choice in acute gout attacks and is almost diagnostic for gout if the drug alleviates the pain. There are other drugs available, primarily the anti-inflammatory class of drugs, but it is important to note that they all do not work in gout. In fact, low doses of aspirin will actually worsen a gout attack. Consultation with your foot specialist is essential.

The pain of gout generally lasts a few days, and then begins to diminish. The problem is when a person experiences multiple gout attacks over short periods of time. Aside from the pain of the attack, each attack does damage to the affected joint and a person can reach the point where there is so much damage from the repeated gout attacks, which the joint now becomes constantly painful and surgical intervention is usually necessary.

In those individuals experiencing repeated attacks of gout it is necessary to find out why. Generally speaking, it is usually from either the body overproducing too much uric acid or the kidneys excreting too little uric acid. About one-third of the uric acid in the body comes from food and alcohol. Here is a list of food that contains high amounts of purines (uric acid is a waste product of purines):

A person who has gout must restrain him or herself from eating food that is rich in purines. But, gout is not caused by food alone. Poor kidney function works in conjunction with the food you eat to build up the uric acid levels in the blood.

How to control Gout

Other than avoiding the foods listed above, another effective strategy to combat the uric acid build-up is to drink plenty of water. Water detoxifies the system, and by increasing urination helps to avoid the crystallization process. Crystallization happens only when the uric acid levels are excessively high in the blood. Ample exercise also helps in reducing gout by increasing the blood circulation in the body.

Certain foods like tofu, nuts and olive oil seem to help a weak kidney in eliminating uric acid from the bloodstream. A diet that has plenty of complex carbohydrates (whole grain bread and pastas, certain fruits, and vegetables) also alleviates the symptoms. If you suffer from high blood pressure, you must make sure the medication you use does not inadvertently increase the uric acid level in your blood.
