Acute Gout

An acute attack of gout is often so typical that the diagnosis is obvious. The onset is often extremely rapid and may reach its worst within an hour or two. One obvious gout joint symptom is the local signs of inflammation in the form of pain, swelling, redness and tenderness. The second attack may not occur for one of two years but will almost certainly happen if the causative factors continue. Recurrences may involve either the same of a different joint. In such a case, the gout joint symptoms are similar to the acute gout attack, such as sudden onset and local inflammation of the joints. After subsidence, a further attack is again likely to occur, but the interval between attacks usually becomes shorter as more acute attacks occur. Other joint symptoms of gout will start to show up on the tip of the elbow between the tendons around the knee, or the Achilles tendon (behind the heel).

Chronic Gout

As more attacks of acute gout occur, remission is likely to be incomplete. At this stage, the following joint symptoms will be observed - urate crystal deposits within the joints are likely to damage the cartilage lining of the joints, interfering with joint function and minimizing the opportunity for repair between acute attacks. By this time, the patient may be in a state of recurrent acute attacks with very little in the way of remissions. In such a situation, inflammation may only be controlled by the constant consumption of anti-inflammatory drugs which limit the response of the inflammatory cells to the crystals. While this may control some joint symptoms, there remains the risk of an extremely severe attack involving many joints (polyarticular gout) unless the underlying cause of the attack is corrected.

For an acute attack, there is no better remedy than a fast. The patient should undertake a fast five to seven days on orange juice and water. He should also avoid all intoxicating liquors, tea, coffee, sugar, white flour and their products, all canned and processed foods. Spices and salts should be used as little as possible. The cherry, sweet or sour, is considered an effective remedy for gout. This was discovered by Ludwig Blan some 45 years ago. Himself a gout sufferer, Blan found the use of cherries to be miraculously effective in his own case and published his own experience in a medical journal. Subsequently, many people with gout used this simple therapy with great success. To start with, the patient should consume about 15 to 25 cherries a day. Thereafter, about 10 cherries a day will keep the ailment under control. While fresh cherries are best, canned cherries can also be used with success. Foods high in potassium such as potatoes, bananas, leafy green vegetables, beans and raw vegetable juices are protective against gout. Carrot juice, in combination with juice of beet and cucumber juices should be mixed in 300 ml. of carrot juice to make 500 ml of combined juice. The juice of French or string beans has also proved effective in the treatment of gout. About 150 ml. of this juice should be taken by the patient suffering from this disease. Raw potato juice and fresh pineapple juices are also beneficial.

Gout occurs in four stages; asymptomatic, acute, intercritical and chronic. The asymptomatic stages are marked by a rise in urate levels. Proper diet also entails drinking between ten and twelve eight-ounce glasses of fluid daily.

The chronic stage is perhaps the worse stage of all. This is when joints are extremely painful because of large crystals that are not only deposited in the joints, but also in between the bones, in membranes and soft tissue.

It is easy to recognize gout because the skin that is covering the crystal deposits develops ulcers that leak. A lesser, but equally painful, symptom is stiffness, leaving the patient unable to move the areas that are affected.

When diagnosing gout, your doctor will order blood tests and x-rays.

The blood tests are to determine the level of uric acid that is in your blood and the x-rays are to find where the crystal deposits are and to determine what the extent of the damage is.

Gout attacks most commonly occur in the foot, primarily in the big toe joint. The next most common locations are the middle of the foot followed by the ankle joint. They can also occur at the attachment of tendons such as the Achilles tendon into the heel. It is possible to have gout attacks in more than one area at the same time.

Diagnosis is generally on clinical appearance and the presenting history because it is not uncommon for a patient's uric acid level to be normal during an attack. Colchicine is the drug of choice in acute gout attacks and is almost diagnostic for gout if the drug alleviates the pain. There are other drugs available, primarily the anti-inflammatory class of drugs, but it is important to note that they all do not work in gout. The pain of gout generally lasts a few days, and then begins to diminish. The problem is when a person experiences multiple gout attacks over short periods of time.
