- Remission of the pain and the fever occurs towards the morning, and the patient wants some rest. However, in severe cases, there is little relief until the second morning when the pain abates but the joint becomes more swollen.

- When the swelling is pressed by the tip of the finger a depression remains even after the pressure is removed.

- During the fever the patient feels heightened thirst and the urine is scanty.

- The pain, fever and restlessness may recur the following night, but again subside in the morning, and thus the disease continues and lasts from five to ten days if not arrested sooner by treatment.

- The swelling usually persists for some time, and the skin peels off, along with constant itching.

- The first attack heralds the return of the disease after a few years, if the patient does not entirely change his manner of living. Afterwards, the attacks become more frequent. Gradually gout can spread to other parts of the body, like the ankles, the instep, the heels and the knees, the wrists and fingers of the hand and the elbows. Several joints may be affected all at once. In severe cases, after the patient has suffered from repeated attacks, even the shoulders, hips or spine can be affected. Gout does not spread from one joint to the next.

- The very first attack may not always commence in the first joint of the great toe, but may sometimes affect the ball of the foot or the ankle, and occasionally one of the fingers of the hand or the wrist.

- Patients suffering from gout are subject to abject heartburn and sour stomach.

Folic Acid ? This is the synthetic version of Folate (a water-soluble B vitamin that is naturally found in dark leafy green vegetables and citrus fruits) that is used in supplements.
Benefits ? Studies have found that high doses of folic acid may help reduce uric acid levels.
Caution ? Extremely high doses of folic acid can lead to digestive stress and may not be safe for pregnant or nursing women, or those who suffer from seizures, kidney or liver disease.

Vitamin C ? This is an essential antioxidant water-soluble vitamin (found in citrus fruits and green leafy vegetables, etc.)
Benefits ? Studies have found that high doses of vitamin C can increase urinary excretion of uric acid, and lower uric acid in the blood.
Caution ? Taking extremely high amounts of vitamin C may change uric acid levels in the body which can result in an acute gout attack. High doses can also cause other side effects.

Bromelain ? A proteolytic enzyme (found in pineapple)
Benefit ? Studies have found Bromelain to be effective in reducing inflammation. It appears to be a more suitable choice over other prescription anti-inflammatory meds used to treat gout.
Caution ? Not recommended for those allergic to pineapples, pregnant women, those with high blood pressure, kidney or liver disease. This nutritional therapy may not be compatible with other medications and can cause side effects such as nausea, vomiting and diarrhea. It is designed for short-term use.

Quercetin ? A bioflavonoid that acts as an antioxidant (found in onions)
Benefit ? Restrains xanthine oxidase, the enzyme that creates uric acid. Studies have found it to have anti-inflammatory affects.
Caution ? More studies need to be conducted to find out the true benefits and side effects of this nutritional therapy.

Other nutritional therapy that is recommended which may prove effective in relieving and preventing gout include:

- Vitamin A
- Vitamin E
- Pantothenic acid (vitamin B5)
- Selenium
- Evening primrose oil
- Fish oil
- Aspartic acid

Keep in mind that you can obtain the above nutritional therapy by naturally incorporating them into your diet, as they exist in many foods.

A lot of gout suffers are interested in following a gout prevention diet. Someone with gout can sometime be the butt of jokes, but it is no laughing matter. It used to be called the disease of kings, as it seemed to effect only royalty and the upper classes, as they were the only ones wealthy enough to eat a meat-rich diet. Nowadays gout can strike just about anyone, which is why a lot of sufferers are interested in following a gout prevention diet to ward off that next attack.

As we have become wealthier, our diets have expanded (along with our waistbands). Gout seems to be a by-product of all this 'good living'.

Gout is a particularly painful form of arthritis that tends to effect the foot and ankle joints and comes on extremely suddenly. Most sufferers are either woken up with a hot, inflamed and painful joint in the middle of the night or wake up in the morning wondering how their joint got to be so painful overnight.

Gout can be painful, debilitating and embarrassing, as friends usually don't understand just how painful a condition it is and the teasing can be pretty harsh. It also can lead to days off work as it can really effect your ability to get around. So unless you have an understanding employer, it pays to find ways to stop further attacks from happening.

There are medications that you can take for the initial attack, but these are aimed at reducing the swelling and taking away the pain rather than tackling the root cause, which is a build up of uric acid in the blood - A condition called hyperuricemia.

Hyperuricemia is not dangerous, however what it can lead to is deposits of uric acid crystals between the spaces and connective tissues in the joints. It is these sharp, jagged needle-like crystals that cause the extreme pain and swelling and explains how gout can come on so suddenly.

Uric acid is produced during the breakdown of purine - a natural compound in our bodies and the food that we eat. Normally our bodies process uric acid fairly efficiently by dissolving it in the blood and excreting it via the kidneys in urine.

Problems occur when the body can't eliminate uric acid quickly enough or production is stepped up. This can be down to eating a purine-rich diet, drinking too much alcohol or becoming dehydrated.

This is where a gout prevention diet comes into it's own. To rebalance your body and get uric acid production and elimination back to normality you should do the following:

1. Lose excess weight.
Around half of all gout sufferers are overweight or obese. Losing the extra pounds will decrease the strain on your kidneys and allow them to process uric acid efficiently.

2. Drink more water.
We all know we should be drinking around 8 - 10 glasses of water a day. Our bodies need this amount of water to function properly. When we don't drink enough water, we become dehydrated and our organs struggle to keep up with all that?s demanded of them. As a gout sufferer staying hydrated is vitally important as it helps to dilute uric acid in the blood, giving the kidneys an easier time of it during the elimination process.

3. Avoid or limit alcohol.
Ideally gout sufferers should avoid alcohol, however if you can't manage total abstinence then at least avoid beer. This contains high levels of purine that will elevate your uric acid levels. Keep all alcohol consumption to the bare minimum and alternate drinks with water.

4. Follow a low purine, low fat diet.
This is the key element to a gout prevention diet. By avoiding foods high in purine and sticking with low purine, low fat options you should return uric acid levels to a more normal level. This should stop crystals from forming and gout flare-ups from reoccurring.

Gout is a disease that is due to an inborn disorder of the uric acid metabolism and is also referred to as metabolic arthritis. When you suffer from this disease monosodium urate crystals are deposited on the articular cartilage of joints and in tissues such as the tendons. Then also causes an inflammatory reaction to the tissue and the deposits get larger and burst through the skin to form sinuses discharging a white material.

The bloodstream usually carries only a tiny amount of uric acid but if there is an increased concentration then crystals are deposited on the cartilage and tissues that surround the joints. If you have high levels of uric acid in your bloodstream it can also result in uric kidney stones.

The common effects of gout include the sudden pain, swelling, redness, warmness, stiffness in the joint, and sometimes a low fever may persist. Those suffering from gout are in pain because of the crystals inside the joint that cause pain whenever the area is moved, as well as the inflammation of the tissues around the joint which tends to cause swelling and soreness.

Gout commonly affects the big toe but can affect other area such as the joints in the ankle, heel, knee, wrist, instep, fingers, and spine. There have been instances where gout appears in the joints of the small toes which have become immobile because of an impact injury which leads to poor circulation and gout.

Hyperuricemia is a uric acid level higher than 420 ?mol/L in males and 380 ?mol/L in females. High uric acid levels don't always lead to gout. If it is suspected that you are suffering from gout the serum urate test should be repeated once the attack has subsided.

To get an accurate diagnosis of gout you must have a light microscopy of joint fluid aspirated from the joint to show intracellular monosodium urate crystal in synovial fluid polymorph nuclear leukocytes. Only a well trained specialist can distinguish the difference from other crystals.

If you aren't able to get immediate medical attention there are a few things that can provide temporary relief to the pain and swelling commonly associated with gout. NSAIDS including ibuprofen can be used to reduce the pain and inflammation, but stay away from aspirin as it has been known to have adverse affects. You can also apply Preparation H ointment to the affected skin to reduce swelling.

You can also apply ice to the affected area for 20-30 minutes multiple times throughout the day. Some may think that uric acid crystallization is increased with low temperatures but a recent study proved that those who use ice packs experienced a relief in pain without negative effects.
