Studies have shown that there is more than one explanation for the mind-deadening agony in the joints, and it is not always due to the high uric acid levels in the blood. This article will give you a run through of what gout is, and what is not.

Gout vs. Pseudo-gout: When gout is not gout

Gout is caused by the crystallization of monosodium urate, or uric acid, in the joints and soft tissues of the body. Generally, this is caused by elevated uric acid levels in the bloodstream and by the body's incapacity to dispose of the excess of uric acid. This acid comes primarily from the food we eat.

In contrast, pseudo-gout, which has essentially the same symptoms (inflammation and pain in the joints), is also caused by the crystallization of chemicals in the soft tissues, but by a completely different chemical. The culprit- calcium pyrophosphate. Unlike in the case of true gout, the calcification of the joints does not cause any pain. What does cause the pain however is the body's natural attempt to destroy the calcium crystals in the joints. Called neutrophils, these microscopic soldiers do destroy the crystals, but in doing so they also destroy the live tissue around. This is the true source of pain for pseudo-gout sufferers. Also, because the crystals in the joints inhibit range of movement, muscles and tendons grow tender.

The difference can be seen clearly under a microscope. Fluid extracted from joints with uric acid crystallization looks very different from fluid with calcium pyrophosphate deposits. This makes the diagnosis easier.

Treatment of pseudo-gout: What to do

Since pseudo-gout is usually a byproduct of post-operative dehydration, patients are advised to drink plenty of fluids before and after surgery. If a patient already has pseudo-gout, nonsteroidal anti-inflammatory drugs or NSAIDs (pronounced en-seds) such as the generic ibuprofen are used to ease pain and swelling. For intense pain, the doctor may opt to inject a powerful corticosteroid directly into the inflamed joint. This will bring immediate relief, though excessive use of this may result in a weakening of the nearby bone that could cause more problems later on.

Few people realize that gout is actually a form of arthritis and an extremely painful one at that. This is why so many sufferers are eager to understand the gout symptoms and solutions available to them.

Uric acid is formed during the breakdown and processing of purine, a compound that is naturally found in your body and also in the food that we eat. As the body metabolizes purine it forms uric acid as a waste product that is eliminated via the kidneys in our urine.

When there is too much uric acid being produced or the body can't dispose of quickly enough, the uric acid stays in the blood stream and is deposited in 'convenient' places, like the joints in crystal form. It is these crystals that cause gout symptoms to flare up.

When a gout attack happens the pain can be pretty intense which sends most people running (or perhaps limping) to the doctor. Commonly NSAID's will be prescribed. These are non-steroidal anti-inflammatory drugs that target the swelling in the joint, although they don't do anything about reducing the uric acid levels. Make sure you ask your doctor about the potential side effects of these.

If the gout attack is very severe a corticosteriod drug may be prescribed instead. Steroids are very effective at bringing pain under control swiftly, however the side effects can be just as dramatic. Make sure you understand the risks involved if prescribed these drugs.

Once you have the pain from the initial attack under control it's time to look at why you had the gout flare-up in the first place.

Some medical conditions can lead to the development of gout, such as untreated hypertension, diabetes, being obese or having high cholesterol. Medications too can increase your risk of developing gout. Low-dose aspirin can increase uric acid levels and some hypertension medications too.

Gout can be hereditary, so take a good look at your family history and be sure to tell you doctor if a close relative suffers with gout. Being a man also increases your risk for developing gout as men have naturally higher levels of uric acid compared to women. This is why when women do get gout, it's usually after the menopause when their estrogen levels are reduced.

Finally gout can be down to your lifestyle choices. Too much alcohol is a common reason for developing gout as is eating a diet high in purine, i.e. lots of red meat, gravies, shellfish and dried legumes.

There are some medications that your doctor can prescribe that reduce the level of uric acid produced by your body. However a healthier way, and what that is more natural, is to make some easy lifestyle changes that allow your body to regain its balance.
1. Follow a low purine diet - swap high protein foods for a more carbohydrate based diet.
2. Lose weight if your overweight, as this can reduce uric acid production. It is best to lose weight steadily rather than do a crash diet as this can temporarily raise uric acid levels, increasing your risk for gout.
3. Limit alcohol or avoid it altogether if you can. Men should limit consumption to two drinks a day, and women one drink a day.
4. Drink lots of water as this helps to dilute the uric acid in your blood and allows the kidneys to eliminate the excess more easily.

A gout attack van be one of the most painful episodes that a person has to endure. They can come on unexpectedly, usually at night, and can leave the effected joint, red, swollen and the sufferer in absolute agony. So can a low uric diet stop gout attacks from striking?

If gout runs in your family or you have had a gout attack in the past, you are at risk of experiencing an attack or having a reoccurrence.

So, what can you do to avoid a gout flare-up? The easiest approach is to look at and adjust your diet to a low uric acid diet.

The problem is that gout is caused by a build up of uric acid in the blood stream. When a specific saturation point is reached excess uric acid is deposited in the spaces and connective tissue of the joints. Gout normally strikes in the big toe or one of the other foot or ankle joints, although it can effect any of the joints in the body. The reason for commonly affecting the feet is because they are the furthest points away from the heart and uric acid crystallizes in cooler temperatures.

Uric acid is formed naturally when the body processes purine, which is found in the tissues of our body and also in many of the foods we eat in a western diet - particularly animal protein.

When we have a diet that is high in purine-rich foods our uric acid levels can go up, which leaving the kidney with a harder job to eliminate the excess uric acid in a timely fashion.

The excess uric acid floats around in our bloodstream until it becomes saturated. At this point uric acid crystals are deposited in the joints.

There are two ways of tackling these high levels of uric acid, either speed up the kidney processes so that uric acid is processed and expelled quickly or reduce the amount of uric acid produced. An effective way to do this is by following a low uric acid diet.

Getting to grips with a low uric acid diet doesn't have to be hard. It boils down to these factors: Avoid foods high in purine and substitute these with foods with a low purine content.

The basic guidelines for this sort of diet is to avoid red meat, processed meat, shellfish and yeast extracts. Instead your diet should consist of gout friendly foods such as, cereals, some green vegetables, fruits and fruit juices, corn and tomatoes.

Chlorinated water may develop risks. Treating Gout

Gout occurs in four stages; asymptomatic, acute, intercritical and chronic. The asymptomatic stages are marked by a rise in urate levels. The Acute stage is marked by a sudden attack of joint pain, swelling, and joins feeling hot and tender, or looking bruised and dusty. This stage only tends to last between five and ten days. The intercritical stage is marked by symptom-free intervals between episodes of gout. Finally, the chronic stage is marked by joints that are persistently painful because of large urate deposits settling on the cartilage, tendons and soft tissues. The skin over the deposits during this stage tends to develop sores releasing a white pus, and those suffering from gout usually experience stiffness of the joints and limited motion of the affected joint or joints.

There is currently no cure for gout, but through the right diet, lifestyle and medications, it is possible to relieve the symptoms and eradicate the risk of further episodes completely.

Proper diet entails avoiding or restricting foods that are high in purine, which is a substance that tends to produce uric acid as it breaks down. Foods that are high in purine include anchovies, sardines, liver, kidneys, sweetbreads, tripe, tongue, mussels, oysters, scallops, peas, beans, lentils and large amounts of red meat. Proper diet also entails drinking between ten and twelve eight-ounce glasses of fluid daily.

A healthy lifestyle entails reducing your consumption of alcohol, and losing weight.

Medicating is a little more difficult, and entails tailoring the treatment to each individual patient. Additionally, it may need to be altered over time. Non-steroidal anti-inflammatory drugs, called NSAIDs may be prescribed to relieve the pain and swelling associated with in acute attack. These medications include colchicines, corticosteroid drugs, and/or adrenocorticotropic hormones, also known as ACTH. In an attempt to prevent the occurrence of future attacks, a doctor may recommend colchicines, probenecids like Benemid, Parbenem or Probalan, or allopurinol.

Lowering risk factors entails a supervised weight-loss program involving exercise, avoiding a purine-rich diet, avoiding alcohol consumption especially in the case of binge drinking, and changing to another drug if currently taking diuretics for hypertension.

Self Care at Home for Gout entails taking all medications as they are prescribed, and staying off or elevating a joint that is hot and swollen. You may want to use a cane or another similar support in an attempt to keep your weight off the joint. If you keep the join elevated as much as possible above your chest, it will aid in letting the joint heal and make the pain lessen significantly.

Surgery is rarely needed for gout, unless the joint damage is significant and has occurred as a result of lack of effective treatment.

Gout is caused by the increase in the amount of uric acid in the blood. Uric acid is normally removed from the body through the urine, however if it is not excreted it can form crystals, which irritate body tissues like the lining of joints. This will in turn cause inflammation and pain, which is often quite severe.

The relief of pain is remarkable with some of the anti-inflammatory medicines also called NSAIDS or non-steroidal anti-inflammatory medications. The typical example is indomethacin or ibuprofen. Applying local ice or heat may sometimes help as well as keeping the joint without moving or bumping into objects.

Colchicine is typically used if the first line medications like NSAID?s do not help but most people have relief of pain ultimately. Even if no medicines are taken the attack will subside in a few days. However if repeated attacks occur there can be lasting damage.

In some situations steroid medications like prednisone may be needed. The steroids will decrease the inflammation and decrease the chemical responses that cause pain. It is important to know that there are many side effects from all of the medications and you must discuss these with your healthcare provider.

In some gout attacks patients may need joint fluid removed for analysis and during the process can have local numbing medicines injected into the joint. These numbing medicines are also called local anesthetics. Sometimes steroids are injected into a joint to decrease the inflammation.

As with all medications make sure you do not have an allergy to them and sometimes taking the medicine with food may decrease stomach upset or gastric distress, especially medicines like steroids.

If a person is on many medications, which can cause gout, changing these may help. One common cause is diuretic medications, which decrease leg swelling or edema. Many older individuals are on these medicines and if they get a gout attack they must remember this fact.
